The updated Annex 1 regulations for sterile medicinal product manufacturing have introduced a wave of compliance requirements that many facilities were simply not built to meet. For manufacturers operating legacy equipment, the path to alignment is neither straightforward nor inexpensive, and the pressure to adapt is exposing deep structural challenges across the industry.

At the core of the problem is a fundamental mismatch between existing infrastructure and the expectations embedded in the revised guidance. Cleanroom design, contamination control strategies, and process monitoring systems that were acceptable under older frameworks now require significant rethinking. Facilities that have operated the same way for years, or even decades, are being asked to demonstrate a level of process rigor that their equipment was never engineered to support.

Legacy equipment creates hard limits on compliance

One of the most direct challenges manufacturers are confronting is the constraint imposed by equipment that predates the current regulatory expectations. As one industry professional put it: "Based on equipment that was not designed under the newest regulations, we are very limited to find ways to implement these Annex 1 changes."

That limitation forces manufacturers into a difficult position. Minor modifications to existing systems may not be sufficient to satisfy the updated requirements, meaning some organizations must evaluate entirely new machine platforms. That is a significant capital and operational commitment, and it often requires rebuilding validated processes from the ground up.

Rethinking processes that have run unchanged for years

Beyond equipment, the challenge is also one of mindset. Many manufacturing processes have remained largely unchanged because they worked, they were validated, and they met the standards of their time. Annex 1 is now requiring facilities to revisit those assumptions. As the same professional noted, manufacturers "have to be able to either go really different ways in machine platforms and rethink processes that we did the same way for a long time" in order to meet the new requirements.

This kind of systemic review is resource-intensive. It demands cross-functional collaboration between engineering, quality, and regulatory teams, and it requires organizations to document not just what they do, but why, and how it controls contamination risk at every step. For manufacturers juggling active production schedules alongside compliance timelines, that is a considerable burden.

The industry is still working through what full Annex 1 compliance looks like in practice across different facility types and product categories. What is clear is that manufacturers who treat this as purely a documentation exercise will fall short. The regulation demands physical, procedural, and technological change, and the organizations best positioned to comply are those willing to critically evaluate infrastructure and processes they may have long taken for granted.
